Some clinics boosted control rates from 44% to over 70%. All it took was support, teamwork, and showing people how to check their numbers at home.
This isn’t just tech — it’s trust. These programs worked best in places hit hardest by health inequality, like Black, Hispanic, and Native communities. With the help of culturally aware health workers, group visits, and pharmacists, people took charge of their health — and it showed in the numbers.
